Indirect full-ceramic restorations are becoming increasingly important. However, they require special preparations with specific materials and fixation methods tailored to the particular requirements of this type of restoration.
To achieve this goal, special preparation guidelines have been compiled for all-ceramic inlays, crowns, and partial crowns, and appropriate preparation instruments have been developed. In addition to traditional preparation methods, the properties of high-strength ceramics such as monolithic lithium silicate allow for new, less invasive preparation styles, reducing damage during grinding and bonding, and thus reducing the risk of pulpitis. This is particularly important from a clinical standpoint because post-procedural pulpitis is a major risk associated with crowns and partial crowns. The German Society for Prosthodontics and Biomaterials (DGPro) discussed the risk of pulpitis in a 2004 publication.
According to the publication, the risk of pulpitis should be controlled by always considering viable alternatives before deciding on crown restoration. If the defect is limited to the occlusal surface, minimally invasive restorations such as occlusal onlays (also known as occlusal veneers or table tops) are a viable option.
However, preparing these restorations with traditional preparation tools is challenging. To facilitate the process, we have developed a new sequence of preparation steps and a new diamond instrument with a suitable shape in collaboration with Lecturer Dr. M. Oliver Ahlers and Professor Dr. M. Daniel.
