Customers will be required to fill in all information according to the information fields in the form available on the website seadent.com.vn/mo-mo-phong-kham such as: Full name, phone number, email, dental clinic name, dental clinic address

1. Purpose of collecting Customer information

– Provide services to customers

– Manage and use Customer's personal information for the purpose of managing the Customer database and promptly handling arising situations (if any).

2. Scope of use of personal information

The website seadent.com.vn/mo-mo-phong-kham uses the information provided by Customers to:

• Provide services to Customers;
• Send notifications about information exchange activities between Customers and Seadent Joint Stock Company;
• Prevent activities that destroy, hijack Customer user accounts or impersonate Customers;
• Contact and resolve complaints with Customers;
• In case of request from competent state management agency.

3. Personal information storage time

There is no time limit; except in the event that the Customer sends a cancellation request to the Board of Directors or the Company dissolves or becomes bankrupt.

4. People or organizations that have access to Customer personal information

The Customer agrees that, if necessary, the following agencies/organizations/individuals have the right to access and collect their personal information, including:

• Board of Directors;
• Third parties whose services integrate with the site seadent.com.vn/mo-mo-phong-kham;
• Competent state agencies in case of request as prescribed in the operating regulations;
• Market research company;
• Financial, legal and auditing advisors;
• The Complainant proves the Customer's violation;
• At the request of competent state agencies.

Customers have the right to check, update, adjust or cancel their personal information by logging into seadent.com.vn/mo-mo-phong-kham and editing personal information or requesting the Administration to do this.

Customers have the right to send complaints about the disclosure of their personal information to a third party to the Board of Directors. Upon receiving these responses, Seadent Joint Stock Company will reconfirm the information, be responsible for answering the reasons and instructing Customers to restore and secure the information.

7. Commitment to protect Customer's personal information

The Management Board is committed to keeping Customer's personal information on the website seadent.com.vn/mo-mo-phong-kham absolutely confidential according to the Personal Information Security Policy posted on the website seadent.com.vn/mo-mo-phong-kham/. The collection and use of each Customer's information is only carried out with the Customer's consent, except in cases where the law provides otherwise and this regulation.

Do not use, transfer, provide or disclose to third parties the Customer's personal information without the Customer's consent, except in cases specified in this regulation or the provisions of law.

In case the information storage server is hacked, leading to loss of Customer's personal data, the Management Board is responsible for notifying and working with the authorities to promptly investigate and handle the matter, and at the same time notify the Customer about the incident.

8. Mechanism for receiving and resolving complaints related to Customer information

When discovering that their personal information is being used for the wrong purpose or scope, Customers send a complaint email to customer-care@seadent.com.vn or call 0901371516 to complain and provide evidence related to the incident to the Management Board. The Management Board commits to respond immediately or at the latest within 24 working hours from the time of receiving the complaint.

In case of failure to reach an agreement, both parties accept the Court as the dispute resolution agency.
